To volunteer with children in the classroom on a regular basis, please complete the application using our online form or PDF:
The following will be completed/required during an intake meeting:
- Copy of valid Michigan Driver’s License or another form of State ID
- Volunteer Emergency Contact Form
- Signed Volunteer Expectations Document
- Completed Criminal Background Check Authorization Form (Online Background Check will be completed)
- Results from TB test
